care for adults and children.

YOUR COMPREHENSIVE EYE
EVALUATION
Your visit at EyeCare Associates will involve checking both vision and
eye health. Using the most advanced equipment, we will do a series of
lens tests to determine if you need glasses and/or contact lenses to
improve sight. At that time, it will be important to know if you have
any special visual requirements for your work, school, hobbies or sports
environments.

The second phase of the eye
health evaluation will involve checking your pupillary responses and
testing your eye muscles for proper coordination. If indicated, a
computerized field of vision test will then be performed to ensure that
you have normal side and central vision.

A thorough evaluation of the
outside of your eye will be done with a microscope and a glaucoma or
pressure reading will be performed. Finally, a dilated examination of
the retina will be done to rule out any eye tissue disease or optic
nerve problems. By dilating or enlarging the pupil, more areas of the
interior of the eye can be seen to better rule out any eye disease. If
necessary, we will take special photographs to monitor for any changes
in your eye health.

EYEGLASSES -
We offer a large selection of quality eyewear, in addition, we carry many
lines of high fashion designer frames. Our professional staff is expertly
trained to help you select frames that look good, fit properly and are
compatible with your lens prescription. We want you to have glasses you are
proud of and enjoy wearing.

CONTACT LENSES

Millions of people have chosen
contact lenses as an alternative to eyeglasses. The primary goal of our
contact lens service is to provide the best fit possible from the wide
variety of lens types and designs available. We also offer a wide range of
specialized services that include lenses for nearsightedness,
farsightedness, astigmatism and for those difficult to fit patients. Most
importantly, we provide complete professional care from the initial
evaluation to the follow-up visits after you receive your lenses.

APPOINTMENTS
- We prefer to see
patients by appointment and emphasize regular, preventive care. We do
understand, however, that eye emergencies can occur, so we will make every
effort to see those patients who have urgent needs as quickly as possible.
These emergencies, on occasion, can disrupt our schedule, but you can expect
us to do our best to honor your reserved appointment time.

PAYMENT AND INSURANCE
MATTERS -
The cost of a comprehensive eye
evaluation will depend on the specific problem and the time and test needed
for a thorough examination. We accept and participate in most medical and
vision insurance. When making your appointment, our receptionist can advise
you if we can assist in filing your claim. In addition, we will happily
estimate your coverage in advance. Payment of your deductible/co-payment is
expected at the time services are rendered. We accept Mastercard, Visa,
American Express and Discover to assist you.

MANAGED CARE
- Over the last
several years we have seen the emergence of a new way of delivering health
care found in the managed care concept. This means that for medical eye care
needs, we sometimes need a referral or authorization from your Primary Care
Physician (PCP) before we can provide any treatment for eye disease. Also,
if you have a medical eye emergency such as a red eye, you must first
contact your PCP and obtain a referral.

Our staff understands and is
familiar with many of these insurances and we will try to remind you to
get the proper authorization before you arrive for your appointment. We
do, however, encourage you to become familiar with your insurance plans
to avoid any unnecessary delays or rescheduling and the inconvenience
this causes both you and our office. It may also prevent delays in our
seeing you in times of emergency.
